What You'll Do (AKA: Your Superpowers)
Lead Resident Support & Advocacy

Oversee and provide social services to residents and families, ensuring individualized care plans address emotional, social, and psychosocial needs.

Serve as a resident advocate, ensuring their voices are heard and respected in all aspects of care planning and decision-making.

Support smooth admissions, transfers, and discharges, including coordinating resources and counseling as needed.

Build & Support a Caring Team

Mentor and support social services staff, volunteers, and interdisciplinary team members.

Provide education and guidance on resident rights, communication, and psychosocial support.

Foster a positive, collaborative environment that supports both staff and residents.

Ensure Compliance & Quality

Maintain compliance with state and federal regulations related to social services and resident rights.

Document assessments, care plans, and progress notes in accordance with policies and regulations.

Participate in audits, quality assurance programs, and care conferences to ensure residents' psychosocial needs are met.

Collaborate Across Departments

Partner with nursing, activities, dietary, and therapy departments to ensure holistic, resident-centered care.

Participate in interdisciplinary meetings and family conferences.

Provide crisis intervention and conflict resolution as needed.

Enhance Resident & Family Experience

Build trusting relationships with residents and families, offering support, education, and resources.

Create programs and initiatives that foster community engagement, belonging, and purpose.

Promote dignity, independence, and quality of life for every resident.

What You Bring to the Table (Besides Your Passion for People)
? Education & Licensure:

Bachelor's degree in Social Work (BSW) or related field required; Master's degree (MSW) preferred.

Current state licensure/certification as applicable.

? Experience & Leadership:

Minimum 2 years of social work experience in a long-term care or healthcare setting.

Previous leadership or supervisory experience preferred.

? Skills & Competencies:

Knowledge of state and federal regulations related to social services and resident rights.

Strong communication, counseling, and crisis intervention skills.

Ability to work collaboratively in a team environment and maintain professionalism under pressure.
