School Based Long Term Services & Supports Care Coordinator (Full Time, Hybrid, North Carolina Based) (Healthcare)



The School Based Long Term Services & Supports Care Coordinator provides comprehensive care coordination and monitoring in collaboration with Wake County Public School System and Special Education Department to provide guidance and planning for students who are identified as needing support with the I/DD eligibility process, Innovations consumers, and consumer's transitioning to or from placement at crisis facilities.

This is a full-time hybrid opportunity. While there is no expectation of coming into the office routinely, the selected candidate must be available to report onsite to the Alliance Office (Morrisville, North Carolina) if needed to attend business meetings. They will also be expected to travel throughout the Wake County area to serve Alliance members as needed.

Responsbilities & Duties

Provide Care Coordination 

  • Ensure clinical assessments of individuals in identified Special Needs Populations are completed in order to identify needs for treatment or monitoring
  • Identify gaps in services and intervene to assure that the individual receives appropriate care 
  • Measure results of intervention and treatment, including reduction in high risk events and inappropriate service utilization
  • Ensure services for the individual are coordinated across the MCO and with other systems, including primary care
  • Provide clinical discharge planning assistance to local hospitals and track individuals discharged from state and local hospitals to ensure they follow up with aftercare services and receive needed assistance to prevent further hospitalizations
  • Ensure individuals identified as special needs enrollees (that have treatment needs or need regular monitoring) have a Behavioral Health Clinical Home and a Medical Home
  • Ensure Person-Centered-Plans (PCP) are developed by a Behavioral Health Clinical Home or, if necessary, by the Care Coordinator to meet urgent needs and to access care for individuals;
  • Verify services are delivered as outlined in person centered plan and addresses any deviations in service
  • Respond to an individual's planned movement outside the MCO geographic area to ensure changes in their Medicaid County of eligibility are addressed prior to any loss of service
  • Monitor documentation to ensure issues/errors are resolved as quickly as possible
  • Ensure all clinical documentation (e.g. goals, plans, progress notes, etc.) meet state, agency and Medicaid requirements
  • Maintain medical record compliance/quality, as demonstrated by ≥95% compliance on Qualitative Record Reviews
  • Collaborate and share information with internal and external stakeholders
  • Serve as a collaborative partner in identifying system barriers through work with community stakeholder
  • Work in partnership with other Partner departments to address identified needs within the catchment area (i.e. System of Care)
  • Develop relationships with community partners to support the population 

 Minimum Requirements

Education & Experience

Bachelor's degree from an accredited college or university in a human service field and two (2) years of full-time, post-bachelor's degree accumulated I/DD experience with the population served

OR 

Bachelor's degree from an accredited college or university in a non-human service field and four (4) years of full-time, post-bachelor's degree accumulated I/DD experience with the population served

 Knowledge, Skills, & Abilities

  • Exceptional interpersonal skills, highly effective communication ability, and the propensity to make prompt independent decisions based upon relevant facts
  • Problem solving and conflict resolution skills
  • Knowledge of behavioral health and intellectual/developmental disabilities
  • Knowledge of the purpose and functions of the Department of Social Services, Department of Public Safety and Department of Probation and the Administrative Office of the Courts
  • Skilled at shifting between macro and micro level planning, maintaining both the big picture and seeing that the details are covered
  • Skilled in Microsoft Office programs
  • Detail oriented with ability to organize multiple tasks and priorities, and to effectively manage projects from start to finish

Employment for this position is contingent upon a satisfactory background and MVR (Motor Vehicle Registration) check, which will be performed after acceptance of an offer of employment and prior to the employee's start date. 

Salary Range

$25.75 to $33.48/Hourly 

Exact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity.

  • Bachelors or better in Human Services
