Responsibilities:

• Coordinate and implement individualized care plans
• Supervise and support LPNs and CNAs
• Oversee admissions, discharges, and ongoing care documentation
• Participate in care conferences and quality improvement initiatives
• Monitor resident conditions and follow up on changes
• Maintain compliance with state and federal regulations
• Assist with infection control, audits, and staff training

Requirements:

• Active LPN or RN license (required)
• Experience in skilled nursing or long-term care preferred
• Strong leadership, clinical, and communication skills
