Job Hours: Times May Vary (8:30AM - 5PM) Monday through Friday - Flexible hours

Base Location: Lawrenceville, GA

Division/Department/Program: Mental Health (MH) Supportive Housing

The Residential Wellness Care Coordinator provides case management & skills training for adults with severe and persistent mental illness & addictive diseases through View Point Healths supportive housing program. This position will focus on the coordination of rehabilitation and recovery services within the agency and in the community for those clients who reside in supportive housing throughout Gwinnett, Rockdale and Newton Counties. Flexibility in scheduled hours is necessary to provide effective in-home services throughout the day and into the evening hours. The vast majority of services provided to clients will be in the clients residence and their community. This position also requires you to work closely with a team of professional staff to assess needs, develop treatment plans, and monitor client progress. This position requires the ability to work in a fast pace setting, a passion to work with those with Behavioral Health needs, and the ability to work closely and effectively with a team of professionals. Reliable transportation and driving abilities are imperative.

Duties & Responsibilities:
Conduct a minimum of 2 home visits per month/per client within their community setting or within clients current residence. Assist licensed staff to provide information to complete Behavioral Health Assessments and update Treatment plans to reflect needed services for authorization. Participate in monthly treatment team meetings to assist in the development of treatment plans with appropriate therapeutic interventions, notify professionals of any unusual observations or incidents, client progress, and follow up. Develops and maintains a network of community contacts. Facilitate referrals and linkages to community services and communication among clients service providers with VPH to assure a continuum of services. Provide linkage, transport clients, and assist/coach clients in accessing & obtaining community resources. Care Coordinators are expected to carry a minimum individual case load of 30 clients. Write precise, accurate, and clear progress notes in our electronic Care Logic system in a timely manner. Maintain required records and information in each clients file. Prepares accurate, complete timely documentation (within 24 hours) of case notes and billing information to meet agency and state standards and provide effective delivery of service across a broad range of services. Provide individual skills training as indicated in the clients treatment plan including but not limited to independent living skills, coping skills, interview skills, symptom management skills, anger management skills, assertiveness skills, advocacy skills, hygiene skills, and many other daily living skills. Recognize the need for crisis intervention and notify supervisor and professional staff of escalated situations.

Programming Specific Duties:
Traveling in Gwinnett, Rockdale and Newton Counties. Perform certain case management duties. Participate in monthly team meetings. Perform other discipline duties when appropriate and needed.

Minimum Qualifications:
Bachelor's degree in a social service-related field from an accredited college or university AND One year of experience in a social service-related field OR High school diploma or GED AND Three years of experience in a social service-related position OR One year of experience at the lower-level Social Services Tech 2 (SST011) or position equivalent.

Preferred Qualifications:
Experience working with the Severe and Persistently Mentally ill population along with individuals with co-occurring substance use diagnoses within a community setting or residential setting. Case management experience Experience working in a highly stressed, fast paced environment with a very emotionally and physically changing population Experience working in multi-disciplinary teams Ability to multi-task and manage time

Requirements/Competencies:
Must be able to lift 20 pounds.Requires long periods of sitting/standing.Must have valid Georgia drivers license and Motor Vehicle Record in accordance with company policy.Candidates for selection must pass a criminal background check (including fingerprinting).Must pass a pre-employment drug screen and subject to random drug screens.Any combination of training and experience which would have enabled the applicant to acquire the necessary knowledge, skills and abilities.

Note: Some positions may require a valid driver's license.
