CNA (Part-Time) - Sherwood Oaks Post Acute (Healthcare)

• Provide daily nursing care to assigned residents in alignment with individual care plans and under supervision.
• Report any changes in resident condition and all incidents/accidents promptly to the Nurse Supervisor or Charge Nurse.
• Document care accurately using only facility-approved abbreviations and maintain confidentiality of all resident health information.
• Assist with ADLs (Activities of Daily Living) such as bathing, grooming, dressing, toileting, feeding, and mobility.
• Ensure residents are comfortable and safe, including frequent checks, answering call lights promptly, and preventing bedsores or injuries.
• Support admissions, transfers, and discharges by preparing rooms, greeting residents, and assisting with personal belongings.
• Record vital signs and intake/output, and report any abnormal findings or changes in eating habits.
• Follow infection control and safety protocols, including exposure to blood/body fluids and proper use of equipment.
• Maintain a positive and calm environment by cooperating with coworkers and creating a supportive atmosphere for residents.
• Must be a licensed CNA in accordance with state laws; CPR certification is preferred.
