Pay Rate: $38 - $42 per hour (DOE)

Willow Creek Healthcare Center is seeking a skilled and compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team. We are a skilled nursing facility committed to providing excellent patient-centered care and fostering a supportive work environment for our staff.
Key Responsibilities:
- Deliver physical therapy treatments as directed by the supervising Physical Therapist
- Accurately record daily treatment notes and weekly progress reports per PT Board standards
- Assist in department upkeep and equipment maintenance
- Participate in care planning and interdisciplinary rehab conferences
- Collaborate with nursing staff to train Restorative Aides
- Communicate effectively with healthcare team members regarding patient care plans and progress
- Conduct in-service training for facility staff as needed
- Educate patients, families, and caregivers in preparation for discharge
- Ensure documentation meets all facility and regulatory requirements
Qualifications:
- Licensed as a Physical Therapist Assistant in the state of California
- Experience with Casamba and PointClickCare preferred
- Strong written and verbal communication skills
- Knowledge of therapy best practices and documentation standards
- Must maintain continuing education per state board guidelines
- Ability to lift/push/pull up to 100 lbs occasionally and meet physical demands of the role
- Ability to relate positively and professionally with patients, families, and team members
