Outreach Worker (401) (Project Management)

Outreach Worker (401)
Fresno, CA

The Outreach Worker I is responsible for providing outreach services in the rural, or unincorporated areas in Fresno County who are experiencing serious mental illness and/or co-occurring substance use disorders and are homeless or at imminent risk of becoming homeless. The Outreach Worker I will work with a high level of independence, engage clients in the field about services and supports available to them, and enroll clients into HMIS (homeless management information system).

How will I contribute
Provide outreach, engagement, and linkage services to homeless individuals struggling with severe mental illness and/or substance abuse in rural and unincorporated areas of Fresno County. "meet persons where they are" and collaborate with other agencies across the system of care to assist persons served in achieving their personal recovery and wellness goals Utilize various engagement tools, such as personal protective equipment, disinfectant supplies, clothing, food, shoes, blankets, bus passes, hygiene kits etc., to address barriers to engagement such as transportation, pet care, property storage, etc. Provide frequent contacts with individuals where they live or are most comfortable in to assist them in accessing behavioral and physical health care, financial, education, vocational, rehabilitative, or other needed community services, especially as these services relate to meeting the persons mental health and housing needs Coordinate person served appointments and transportation as required Maintain collaborations and supportive relationships with other agencies Maintain clear, accurate and concise consumer records, in a timely manner. Assist with program design and implementation. Effectively document services provided into HMIS Database. Perform other duties as assigned.

What I bring
High school graduation or equivalent plus one year of related experience, or B.A. degree with no experience in related field from a college or university Class C Driving License Local resources, information about accessing social welfare systems, advocate on behalf of clients Computer skills a must. PATH utilizes HMIS database to document services and progress. Ability to relate to others comfortably and be able to establish and maintain effective relationships. Ability to communicate effectively in writing. Excellent time management. Ability to multi-task Great people skills, patient and persistent. Have a positive attitude toward disadvantaged and disenfranchised individuals. Able to think and work independently to make decisions when faced with daily outreach challenges. Motivational interviewing skills experience preferred. Experience engaging and linking homeless individuals struggling with severe mental illness and/or substance abuse to services.
