Speech Language Pathologist (SLP) - Sherwood Oaks Post Acute (Healthcare)

• Evaluate patients within 24 hours of physician referral or document reason for delay in the medical record.
• Develop and implement treatment plans with physician approval and provide therapy per the plan.
• Supervise Speech-Language Pathologist Assistants and CFY/Clinical Fellows, ensuring compliance with state practice acts.
• Communicate regularly with supervisors, health team members, and participate in care conferences and utilization reviews.
• Complete all required documentation, including evaluations, daily notes, weekly progress, and recertification per regulatory standards.
• Provide caregiver training and family education for discharge planning and maintenance programs.
• Participate in in-service training programs and act as a clinical preceptor for students and new clinicians.
• Maintain active state licensure, meet continuing education requirements, and comply with all state and facility policies.
• Demonstrate strong interpersonal, communication, and problem-solving skills with patients, families, and staff.
• Meet the physical demands of the job, including frequent standing, walking, lifting, pushing, and pulling, often with moderate to heavy exertion.
