What You'll Do:
- Evaluate and treat post-acute patients with individualized care plans
- Collaborate with other therapy professionals, nursing staff, and families
- Promote functional recovery, independence, and mobility
- Document progress and outcomes according to facility standards
Requirements:
- CA-licensed Physical Therapist (or eligible)
- SNF experience a plus, but new grads welcome!
