Referral Coordinator - Dunn Behavioral Sciences Center/HCPC at UTHealth Houston (Finance)



Position Summary:

Referral Coordinator and Milieu Specialist will handle the Intensive Outpatient (IOP) and Partial Hospitalization (PHP) Programs. This role will play a crucial role in the initial clinical screening of patients to determine appropriateness of fit, and suggested level of care for our programs. The Referral Coordinator will work closely with the Directors of Eating Disorders, Co-Occurring, and Affective Disorders Stepped Care Programs to facilitate the screening process, using evidence-based measures to support entry into programs.

Position Key Accountabilities:

Essential Functions

  • Provides indirect services to patients. Acts as an advocate for the patient/family with the treatment team.
  • Provides direct services to patients including: brief clinical interview/screening, recommendation of level of care, and milieu support.
  • Provides oversight of all incoming referrals and facilitates rapid entry to programming for those who meet criteria. Collaborates with reception staff and other providers to facilitate scheduling.
  • Works with a multidisciplinary treatment team to develop an individualized treatment plan. Works closely with physicians in identifying diagnoses to assist in the treatment process. Develops a treatment plan that will support patient 's ongoing recovery and development.
  • Maintains in-depth knowledge of the various OP/IOP/PHP programs and provides education to patients and referring providers on services offered.
  • Maintains in-depth knowledge of inpatient services at the Dunn Behavioral Sciences Campus and collaborates with inpatient treatment teams to support transition of step-down or step-up care.
  • Establishes and maintains relationships with community resources to assist with patient's medical needs for on-going treatment, residence, financial support, transportation, education, and vocational rehabilitation.
  • Refers patients and family to community resources if patients are not accepted to programming based on screening.
  • Provides group cross-coverage when needed utilizing manualized treatment modalities.
  • Performs other duties as assigned.

Certification/Skills:

  • Strong written and verbal communication skills and/or strong interpersonal communication sills.
  • Proficient in Microsoft Office Suite applications including Word, Excel, PowerPoint and Outlook.
  • Advanced skill level in Excel.
  • LCSW- License Clinical Social Worker required or
  • LPC-Licensed Professional Counselor required

Minimum Education:
Master's Degree in Behavioral Sciences required
Minimum Experience:
1 year in mental health setting required
Physical Requirements:
Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently and/or a negligible amount constantly to move objects.
